The Jan. 5, 2021, data security incident at Granite Wellness Centers exposed their personally identifiable or protected health information. They received a notice of the data security incident from Granite Wellness Centers on or around March 6, 2021, or believe the breach affected their data and can verify their identity and class membership. The types of personal information potentially exposed include names, dates of birth, home addresses, dates of care, treatments, treatment providers, health information, health insurance information, driver’s license numbers, medical histories, Social Security numbers and bank account numbers.
