All complaints and other communications sent to the Department are public records under Florida law and may be subject to disclosure upon request. In order to assist us in evaluating your complaint, please provide: The name, address, and phone number of the complainant. The name of the victim or other witness of the alleged discrimination, if it is someone other than the complainant, if known. The basis for the complaint. Please include as much detail as possible. The date of the discriminatory conduct. The name and title of the person(s) who is alleged to have engaged in the discriminatory conduct. The ADA Complaint Coordinator will provide written notice to the complainant of the receipt of t
